Your name as you want it to appear on the diploma…be very specific! Whatever you list here, is what will be printed on your diploma. Questions to think about: Do you want your middle name included? Do you want your middle initial with a period or without a period? Do you want a suffix included? *If your name has changed due to marital status, you must fill out a “Change of Information” form, and provide required documents before the new name will be printed on your diploma. You can find this form under “Alumni Resources” on the school website, or visit the Academic Office to fill it out.
